DOCUMENTOS QUE ACOMPAÑAN LA SOLICITUD

(List of documents attached) Documentos que acompañan la solicitud de restitución internacional:

1. Formulario Haya debidamente diligenciado, (si es diligenciado en letra manuscrita, debe ser clara y legible). Es muy importante aportar la mayor cantidad de datos de posible ubicación y/o contacto del niño, niña o adolescente, tales como teléfonos, dirección, ubicación de familiares, que permitan la localización del menor de edad.

2. Escrito de motivación del aplicante, a través del cual, de manera detallada y precisa, explique las razones por las cuales aplica a una solicitud de Restitución

Internacional.

3. Copia autentica o en caso contrario fotocopia del documento de Registro de nacimiento del niño, niña o adolescente (en caso de tener doble nacionalidad es prudente aportar copia de ambos documentos).

4. Copia del documento de identidad del aplicante (en caso de tener más de una nacionalidad adjuntar copia de cada uno los documentos de identidad).

5. Copia del permiso de salida del país del niño, niña o adolescente.

6. Copia del documento de identidad de quien sustrajo o retiente ilícitamente al niño, niña o adolescente (no es obligatorio).

7. Fotografías recientes del niño, niña o adolescente y de la persona que presuntamente sustrajo o retiene al menor de edad.

8. Se recomienda aportar la mayor cantidad de documentos que acrediten la residencia habitual del niño, niña o adolescente como: certificación y/o documentos que acrediten la vinculación educativa, vinculación a salud, a clases extraescolares

(arte, deporte, cultura) copia de carné de vacunas y demás documentos que

acrediten la residencia.

9. Documentos que regulen la Custodia y Cuidado Personal del niño, niña o adolescente.

10. Si la solicitud va dirigida a otro país de no habla hispana, absolutamente todos los documentos (incluyendo documentos de identidad); deben ser al idioma oficial del país requerido o a uno de los idiomas oficiales del Convenio (inglés o francés). NO se requiere una traducción oficial, pero se recomienda una buena traducción.

12 SUBDIRECCIÓN DE ADOPCIONES Las solicitudes serán objeto de análisis y revisión por parte de la Autoridad Central Colombiana y sujetas a ampliación de acuerdo con cada petición. Attached documents of the request of return of the child:

1. Request form duly completed. It is very important to provide information concerning the location where the child is thought to be, such as: phone number, address and location of family members.

2. Applicant's motivation brief explaining the reasons to request the return of the child

wrongfully removed or retained.

3. Original or certified copy of the birth registry of the child (In case of dual nationality, please provide a copy of both documents).

4. Copy of the identity card of the applicant (in case of having more than one nationality, attach a copy of each identity card).

5. Copy of the permission given to the child to leave the country.

6. Copy of the identity card of the person who wrongfully removed or retained the child

(not mandatory).

7. Recent pictures of the child and of the person who wrongfully removed or retained the child.

8. Please provide detailed information proving the habitual residence of the child, such as: school certificates, health insurance, certificate of attendance to extracurricular classes (art, sports, culture), certificate of vaccination and other documents proving the habitual residence.

9. Documents that regulate the Custody and Personal Care of the child.

10. If the application is addressed to a non-Spanish speaking country, all documents

(including identity documents) must be translated to the official language of the requested country or to one of the official languages of the Agreement (English or French). An official translation is NOT required (Article 23 and 24 of the 1980 Hague Convention).
